Community Services

St Vincent’s Community Health Service is a multidisciplinary non–emergency service offered to people living within the Inner City, Waverley and Woollahra areas.

Services include:

• Nursing
• Physiotherapy
• Occupational therapy
• Social work
• General counselling
• Podiatry
• Post acute care
• Chronic care
• Transitional aged care
• A day centre for dementia care
• Health promotion activities

The nursing services offered include, generalist, dementia, heart failure, homeless outreach, HIV/AIDS and post acute care nursing.

No referral is needed to access Community Services, however a health assessment is undertaken to determine whether Services are required.
