Geriatric Medicine at St Vincent’s
Geriatric Medicine covers all aspects of medical and psychosocial problems in the frail, older patient group, in both acute and sub-acute settings, as well as the community. Our Acute Geriatric Service covers the acutely ill who are brought to hospital via the ambulance, and is situated in St Clare’s Ward, Xavier 9 North. This ward has 34 beds, 10 of which are designated as the Medical Assessment Unit, which is especially staffed to undertake rapid, multidisciplinary assessments as an alternative to admission via Emergency Department.
In addition, we run a range of programs designed to improve transitions of care between the home to hospital and back home again, including the Transitional Aged Care Program (TACP) and the AgedCare Services Emergency Team (ASET).
Non-Acute Services
Our non-acute services are mostly based at War Memorial Hospital and include inpatient geriatric rehabilitation, outpatient services with a range of specialist clinics such as Cognitive Assessment and Treatment Clinic, Parkinson’s Disease Multidisciplinary Assessment Clinic and Falls Clinic. Geriatric Medical clinics also operate from St Vincent’s outpatients. Geriatric and psychogeriatric consultations are provided for the entire St Vincent’s campus as well as surrounding hospitals, and domiciliary geriatric assessments and ACAT assessments are available on special request.
How to Refer
All inpatient consultations and outpatient specialist medical clinics require a doctor’s referral. However, families are encouraged to seek a referral from the treating specialist team if they feel that aged care issues, including ACAT assessment, need to be addressed. Clinic appointments can be made through our offices at St Vincent’s Hospital and War Memorial Hospital.
