Palliative Care Program

Sacred Heart Palliative Care provides an integrated interdisciplinary palliative care service to patients with life-limiting illness. Pain control, symptom management and pastoral support are some of the key services provided by the team. Patients and carers are supported by a

• 50 bed inpatient unit
• Day Centre (operates 5 days a week)
• Community Palliative Care Team operates 5 days/week (8:30am - 5:00pm)
• Weekends and overnight an on-call service provides emergency cover

A consultation service is provided to St Vincent’s Hospital, St Vincent’s Private Hospital and Prince of Wales Hospital.

All of the services cover patients living in the northern sector of SESIAHS but also accept referrals from surrounding areas subject to clinical assessment.

How to Refer	Patients can only be referred to Sacred Heart via their  local palliative care service or, for patients living  at home, by their GP.
